If your out-of-pocket cost for GARDASIL is over $30, you may be eligible to receive a partial rebate.*
The amount of any rebate will be the out-of-pocket cost for GARDASIL minus $30. No rebate to exceed $130.
Because everyone's health insurance is different, your rebate may not be the same as someone else's rebate.
Here are some examples of how it's calculated:
- The Explanation of Benefits (EOB) document indicates that the out-of-pocket cost for the dose of GARDASIL administered is equal to or less than $30. This means you are not eligible for a partial rebate. For example, the EOB indicates that you have a $20 co-pay for the dose of GARDASIL administered. Because the out-of-pocket cost is less than $30, you are not eligible for a rebate.
- The EOB document indicates that the out-of-pocket cost is more than $30 but less than $160. This means you are eligible for a rebate.
For example, the EOB indicates that your out-of-pocket cost for the dose of GARDASIL administered is $100. Because the out-of-pocket cost is more than $30 but less than $160, you are eligible for a rebate of $70 ($100 minus $30).
- The EOB document indicates that the out-of-pocket cost is more than $160. This means you are eligible for a maximum rebate of $130.
For example, the EOB indicates that your out-of-pocket cost for the dose of GARDASIL administered is $175. Because the out-of-pocket cost is more than $160, you are eligible for a rebate of $130 ($175 minus $30 = $145; however, the maximum rebate is $130).

GARDASIL is the only human papillomavirus (HPV) vaccine that helps protect against 4 types of HPV. In girls and young women ages 9 to 26, GARDASIL helps protect against 2 types of HPV that cause about 75% of cervical cancer cases, and 2 more types that cause 90% of genital warts cases. In boys and young men ages 9 to 26, GARDASIL helps protect against 90% of genital warts cases.
GARDASIL may not fully protect everyone, nor will it protect against diseases caused by other HPV types or against diseases not caused by HPV. GARDASIL does not prevent all types of cervical cancer, so it’s important for women to continue routine cervical cancer screenings. GARDASIL does not treat cervical cancer or genital warts. GARDASIL is given as 3 injections over 6 months.
IMPORTANT SAFETY INFORMATION
Anyone who is allergic to the ingredients of GARDASIL, including those severely allergic to yeast, should not receive the vaccine. GARDASIL is not for women who are pregnant.
The side effects include pain, swelling, itching, bruising, and redness at the injection site, headache, fever, nausea, dizziness, vomiting, and fainting. Fainting can happen after getting GARDASIL. Sometimes people who faint can fall and hurt themselves. For this reason, your health care professional may ask you to sit or lie down for 15 minutes after you get GARDASIL. Some people who faint might shake or become stiff. This may require evaluation or treatment by your health care professional.
